    Many people have a weak shoulder without knowing it, until an everyday movement – such as lifting a suitcase – unleashes pain or stiffness. The rotator cuff handle is made up of four tendons that stabilize the joint: if they don’t stay strong, they are easily injured.

    This article gives you a safe, progressive routine for strengthening your rotator cuff and protecting your shoulder, especially if you already have signs of wear and tear or a history of injury.

    Before starting the exercises, carry out a self-assessment:

    A. External Belt Rotations (Basic to Intermediate Level)

    1. Hold an elastic band with both arms close to your body.

    4. Perform 3 sets of 15 repetitions per arm.

    B. Isometric Front Lift (Basic Level)

    1. Rest your forearm against a wall with your elbow at 90°.
    2. Press against the wall without moving your arm.
    3. Hold for 10–15 seconds. Repeat 5 times per arm.

    C. “Y” lying face down (intermediate level)

    1. Lie on your stomach in a neutral position.
    2. Raise both arms in a “Y” shape while keeping thumbs towards the ceiling.
    3. Hold for 2–3 seconds, slowly lower.
    4. Do 3 sets of 12 reps.

    D. Low row with band (intermediate level)

    1. Attach a band to your chest.
    2. Take the band, extend your arms and pull towards you, squeezing your shoulder blades.
    3. Perform 3 sets of 15 repetitions.

    E. Internal Belt Rotations (Advanced Level)

    1. Attach the band to the opposite side.
    2. Turn your forearm inward across your body.
    3. Perform 3 sets of 15 repetitions per arm.

    4. Progression and frequency

    • Do this routine 3 times a week.
    • Increase the resistance of the band or the duration of isometrics every 2 weeks.
    • If you feel sharp pain, reduce the intensity or consult before continuing.

    5. When to seek medical attention

    You should see an orthopedist if:

    • Pain does not improve after 4–6 weeks of exercise.
    • There is significant loss of strength or disabling stiffness.
    • Frequent clicking or instability in the shoulder appears.

    6. Additional recommendations

    • Warm up earlier with gentle mobility and pectoral stretches.
    • Complement with upper back exercises (middle and lower trapezius).
    • Remember that the cuff acts in synergy with postural muscles.
    • Maintain good posture: avoid elevated or hunchbacked shoulders.
